Case management is a collaborative process which assesses, plans, implements, co-ordinates, monitors and evaluates the options and services required to meet an individual’s health, social care, educational and employment needs as defined by the Case Management Society UK (CMSUK.)
Case Managers support clients and their families and the level of support can vary drastically depending on different circumstances, such as the level of physical injury, level of brain injury, mental capacity issues, stages of life when the accident/injury may have occurred and the support which family/friends may provide.
Brownbill Case Management can adapt their approach according to clients’ need and can offer full case management support or work with family members to offer support in key agreed areas.
Where minimal support is needed this is often referred to as a Light Touch Case Management Service. You may want to direct your own support or have family members assist with this and just need support at certain times, for example to assist with organising a holiday or finding a new therapist.
However, if you have support staff who assist with personal care and you would like Brownbill Case Management to oversee this care and supervise your support staff, you will need to be aware of our requirements under our registration with the Care Quality Commission (CQC).
